As part of the MAPP2Health Community Health Assessment, the Blue Ridge Health District is organizing a community health survey! They’ll be swinging through several neighborhoods in Nelson County from May-August 2024 to ask some quick questions on your health and well-being. Please help with participating in this survey so the voices of Nelson County can be heard. The survey will take place in-person on your front porch / outside of your house by a trained interviewer. Responses will be confidential and the survey should take approximately 10-15 minutes to answer. Complete the survey and receive a $25 gift card!
